Dr. Clock tries to reconcile J.D. and Eliot, while waiting to see which one Dr. Cox will give the Chief Resident position to. Molly and Carla are experiencing some subtle conflict and Elliot and J.D. begin to come together after long bouts of conflict of their own. Meanwhile, Dr. Cox, Turk and the Janitor are forced to work together to help a patient.

Für lange Zeit sah es so aus, als wäre J.D. der Posten als Chef-Assistenzarzt schon sicher - schließlich hatte er keinen Mitbewerber. Dann bekommt Dr. Cox doch noch einen zweiten Anwärter zur Auswahl: Elliott, die in letzter Sekunde von Molly dazu gedrängt wird, sich auch zu bewerben. Pourvue de bonnes intentions, elle tente de réconcilier JD et Elliot, en compétition pour décrocher le poste de Chef des internes. Nel frattempo il figlio di un benefattore dell'ospedale viene ricoverato a causa di una lampadina nel retto e il suo caso passa di medico in medico senza però trovare una soluzione concreta. Solo grazie all'aiuto dell'inserviente, il Dr. Cox e Turk riescono a trovare il rimedio migliore, di cui si prenderà però i meriti il Dr. Kelso.
